First Lady (ファースト・レディ, Fāsuto Redi?) is the archetype of all Magical Girls. She is the primary antagonist of the Fate/Grand Order Prisma Causeway event.

Background[]
She was born in the Demon World but was raised among humans and fought for them as the first Magical Girl.[1] She became best friends with a girl named Mila but was forced to kill her when Mila became the first corrupt Magical Girl.[2] Due to her hiding her identity, she was forgotten by both humans and her brethren following her death and passed into legend as the First Lady, the first Magical Girl.[1]
Eventually, her regret over killing Mila triggered the formation of a Reality Marble. As well, her desire to spread the Magical Girls' magic of love and throughout the world attracted Echoes, the spirits of failed Magical Girls, from various worlds.[1] Their despair and obsession to remain as Magical Girls became a flow of magical energy. Using this flow, the First Lady formed and sustained the Reality Marble as an Echo.[3]
Personality[]
She very much enjoyed being a Magical Girl and saving the world as one. However, people's greed and dissatisfaction wore her down to the point where she cried while smiling. So when she was abandoned and forgotten as a mere legend, the First Lady felt utterly betrayed. This and the regret over killing Mila warped her desire for Magical Girls to continue saving the world into a selfish obsession.[1]

The silhouette used to portray First Lady is from the Illya Prisma Cosmos Craft Essence. Her true appearance is unknown as she is only seen possessing Chloe von Einzbern, with her presence being indicated by glowing purple eyes.

Event: Prisma Causeway[]
As part of her plan to grant her wish for Magical Girl to continue saving the world, the First Lady gave the Echoes temporary bodies and magical energy.[1] She then instigated the Magical Girls[4] into killing each other to try and establish their own kingdoms.[5] Of the survivors, she chose the strongest of them, Connacht ☆ Medb, to be her servant, whom she had gather more Magical Girls.[6] However, none could grant her wish because they weren't real Magical Girls. Thus she determined she needed a real one. That was when she met Chloe von Einzbern[1] through a familiar[7] and made her into her vessel.[1] She then found a way to peer into parallel worlds and found the Magical Girl ideal for her plan, Miyu Edelfelt.[8][1] So while Miyu and Illyasviel von Einzbern were headed for the Mirror World, the First Lady ambushed and brought them into the Reality Marble.[9][8] She then took advantage of Medb's attack on the two to capture Miyu while disguised as a familiar of Medb's.[10] She then imprisoned Miyu in her castle and restrained her in the Anti-Magical Girl Mystic Code, Voile de Deuil, for the purposes of harvesting her magical energy.[8][7] The First Lady eventually turns Chloe against Illya by calling her an accessory to Ilya and using her desire to have Miyu to herself.[7]
When Ritsuka's party nears her castle, the First Lady promises to give Chloe back control of her body until after the latter's duel with Illya. She fulfills her promise and retakes Chloe's body almost immediately following the duel's conclusion. Afterward, she introduces herself to Ritsuka's party and explains her past and circumstances. She declares that with Miyu's power she will incarnate the Echoes into Magical Girls and spread them across parallel worlds to save people. She’ll soon absorb Miyu as Chloe resists assimilation with her. The group calls her wish selfish, with Illya saying it will only turn into a curse. But the First Lady refuses to consign herself to the past. She suspects Ritsuka is the type of person who wishes to be forgotten and believes they would die when they save the world. She then asks Mash if she could endure Ritsuka abandoning her and if she'd cling to the hope she could be with them forever. Mash says that isn't hope and that she respects the First Lady but she doesn't pity her. Considering it a miracle they met, she will not forget her, even if the First Lady forgets her. The First Lady calls her a liar. Chloe briefly takes back control of her body before the First Lady retakes control. The First Lady reveals her and Chloe’s desire for Miyu allowed her to break through the latter’s mental wall and fully control her body. Once Miyu is emotionally broken by seeing the group dead before her, she’ll use Miyu to reach infinite parallel worlds. The group then fights the First Lady to save her from herself.[1]
With the First Lady defeated, Chloe slowly regains control of her body, and Miyu is freed. Urged by Miyu, Illya confesses to the First Lady that she would be overwhelmed if she had the power to save the world. But if her friends and family are in danger, she will use her magic without hesitation. The First Lady sees her old self in Ilya and recalls how she killed her best friend to protect the world when she refused to be a Magical Girl. Mahatma ♀ Helena then arrives and takes everyone outside.[11]
There Magical Ruby has the passive Echo possess Illya. The possessed Illya immediately starts hitting the First Lady, calling her an idiot. The First Lady realizes from her speech that she is Mila, surprised she is an Echo. Mila confirms she is as she was a Magical Girl if only briefly. She confesses she chose the First Lady over the world as the people caused her friend despair with their greed and dissatisfaction. The First Lady asks Mila if she was always in the Reality Marble but she never noticed. Mila confirms this, saying she always felt the First Lady's presence as the Reality Marble is the manifestation of her regrets. Because of that, she always wished to go beyond the black wall, so she may bring peace to her friend's heart. This wish inadvertently summoned Chloe to the Reality Marble. The First Lady reveals Medb was once stronger than her, using her vast magical energy to open a leyline to Chaldea. She then begins to disappear, causing her Reality Marble to start collapsing. Helena convinces the First Lady to give her ownership of the Reality Marble so it can serve as a refuge for wounded Magical Girls to stop them from becoming like the First Lady. Mila tells the First Lady that Magical Girls seek no rewards, nor do they use their magic for themselves. They merely put their innermost feelings into theirs spells and cast them for people they don't know. Otherwise, their impossible wishes become curses and their emotions get trapped between worlds and become purgatory. Mila, therefore, convinces the First Lady to stop and leave the future to the next generation of Magical Girl. The two friends then disappear together.[2]
Abilities[]
Like the King of Heroes originates the existences of many heroes who illuminate human history, the First Lady originates the concept of Magical Girl. By this logic, all other Magical Girls are mere imitations that have been copied continuously. So compared to the First Lady, they would be at a great disadvantage in magecraft and the like.[8]
It is suspected by Magical Ruby that the First Lady has power over Magical Girl as she is able to keep Miyu Edelfelt in her Magical Girl form even though Magical Sapphire was a fair distance away from Miyu.[8] The First Lady's power is further demonstrated by the fact she can grant Echoes temporary bodies and magical energy so they may act as Magical Girls once again.[1] She can also disguise herself as other Magical Girls' familiars.[10]
She possesses Voile de Deuil, an Anti-Magical Girl Mystic Code. Appearing as lace ribbons, it restrains a Magical Girl and drains her of her magical energy. It also has a defensive function where parts of it will turn into hostile familiars if anyone nears it.[7]
